YEARLY COMPLIANCE REQUIREMENTS

1. Medically cleared by Health Services for participation in athletics (Link to Health Services Website for forms below)
2. Complete the four (4) NCAA Yearly Compliance Forms and watch accompanying NCAA videos. Student Athletes will receive an email from Teamworks Compliance to your WNE E-Mail account to access the four (4) listed forms.
   1. NCAA Compliance Questionnaire
   2. NCAA Drug Test Consent
   3. NCAA Student-Athlete Statement
   4. NCAA Summary of Regulations
3. Attend WNE Compliance meeting
4. Attend Mandatory NCAA Title IX Training
5. If you are an international student athlete, in addition to the above steps, you will need to obtain an NCAA Amateurism Certification. Details on how to complete that process can be found in the following link.
